Massage therapy is not a substitute for medical examination, diagnosis, or treatment.

It is my responsibility to inform Danielle of any known medical conditions, injuries, or changes to my health prior to and during the session.

I understand that massage may cause temporary soreness, tenderness, bruising, or fatigue, which are normal responses to therapeutic bodywork.

I acknowledge that Danielle will assess whether massage is appropriate based on my current health and may cancel or discontinue a session if necessary for health and safety.

I affirm that I have disclosed all relevant health information truthfully and will provide updates as needed.

I voluntarily assume all risks associated with massage therapy and release Danielle and Danielle’s Table, LLC from any liability for injury or conditions that may result, except in cases of gross negligence.
