I’m honored to be an in-network provider with Providence, Regence Blue Cross, and PacificSource Insurance.

To Schedule an Insurance Massage

Please email the following information to danielles.table@gmail.com:

Your Full Name and Date of Birth
Your Insurance Provider, Policy ID Number, Group Number, and Insurance Phone Number

Coverage & Session Options

If you know you have massage benefits, feel free to schedule a 60-minute session.
If you prefer a 90-minute session, I will bill your insurance and you will be responsible for your copay plus the cost of the additional 30 minutes.

If pre-authorization is required by your plan, please let me know in advance so I can obtain it before your first session.

Important Notes

If your insurance does not cover the session, and you’ve agreed to move forward with treatment, you are responsible for full payment.
