I am honored to be able to accept Providence, Regence Blue Cross and Pacific Source Insurance.
I bill insurance $140 and they pay based on the agreed upon amount in my contract with them.

If you know that you have Massage coverage, schedule your 60 minute Massage. If you prefer a 90 minute Massage I will bill you for your co-pay plus the cost of a 30 minute Massage.
If you are a No Show or make a short notice cancellation for your Massage, you will be responsible for the Full amount of the Massage.
If you need pre-authorization please let me know so that I can have your sessions pre-authorized before I see you.
If for some reason your insurance policy does not cover the cost of your massage and you agreed to have me work on you knowing that the sessions may not be covered by your insurance company, you are responsible to pay for your session(s) out of pocket.
